Dividends are not guaranteed.
Member’s Eligibility
– 1 to 70 years old(age as of the nearest birthday)
– 56 and above are required to undergo the medical exam
Juvenile Insured
– Minor member who is covered by the plan
– 0 to 17 years old
– Payor should be 18-60 years old
